SPITEX Grauholz is responsible for the high-quality, reliable, and needs-based care of 11 municipalities in the field of outpatient nursing and domestic services. As a progressive organisation, we continuously develop and rely on our 180 competent, open-minded, and motivated employees. They work in one of the three bases Jegenstorf, Münchenbuchsee, Urtenen-Schönbühl, or in the Psychiatry and Assessment Team.
To strengthen our assessment team, we are looking for an immediate or agreed start:
Dipl. Nursing Specialist HF/FH/AKP as Assessment Specialist
You want to
- work independently and experience insight into various households while still having a supportive and appreciative team behind you.
- clarify the required care for clients and competently advise their relatives from registration to handover to the care teams (including creating the initial care plan).
- plan and conduct reassessments for existing clients (if necessary, also occasionally with short care assignments).
- participate in a varied work environment and support people in both simple and complex care situations in various households.
You bring
- a completed education at HF, FH, or AKP level with 2 years of professional experience.
- motivation and joy in working with people of all ages in an outpatient setting (preferably experience in outpatient care).
- strong communication skills as well as high social and self-competence.
- in-depth knowledge and enthusiasm for the care process.
- the basic InterRAI HCSwitzerland course or willingness to complete it.
- ideally additional training (e.g., in Palliative Care, Gerontology, or as a Wound Expert).
- high commitment and enjoyment of flexible and independent work.
- the ability to efficiently and conscientiously handle administrative tasks and ideally know our industry software Swing. You also possess a category B driving licence.
