In this responsible role, you will take on independent case management and ensure needs-based, high-quality care and support in the home environment. To strengthen our somatic team at Spitex Seewadel, we are looking for a committed individual to start immediately or by arrangement as:
Certified Nursing Specialist Spitex with Case Management (70–100%)
Your core tasks • Responsibility for case management including conducting assessment and counselling interviews using InterRAI-HC assessments • Planning, implementation and evaluation of care processes • Ensuring needs-based and client-oriented care in the home environment as well as adjustments to changing situations • Interprofessional collaboration with general practitioners, relatives, institutions and other professionals • Active participation in quality assurance and further development of the somatic department Your skills • Completed training as a nursing specialist HF/FH with experience in case management and care process control (RAI-HC) • Sound expertise in the care and support of people in complex situations in the outpatient setting • High social competence with empathetic and appreciative interaction, professional relationship building as well as person-centred counselling and support • Very good German language skills in spoken and written form as well as proficient PC user skills • Strong communication and teamwork skills as well as a high degree of independence, sense of responsibility and resilience Your advantage • A challenging role in a motivated team of a dynamic organisation • A positive working atmosphere with attractive employment conditions and social benefits as well as above-average remuneration • Internal and external training opportunities, supervision • A workplace near the station with good connections to public transport Are you interested in joining a forward-looking company?
