The position is limited from 1 December 2026 to 31 December 2027, with the option of extension.

Your field of activity

• You advise patients with heart failure in the heart failure counselling team within the scope of your advanced nursing competencies and actively help shape the further development of this service within the LUKS Group
• You care for patients with telemedical care components together in the interprofessional team of integrated care with cardiologists, general practitioners and Spitex professionals
• You take on a central coordination role in this new care system with telemedical components and actively help shape its development and implementation within the framework of a study
• You are actively committed to the further development of nursing care for patients with heart failure within the LUKS Group and beyond

Your experience

• You have a completed master's degree in nursing as an APN, preferably towards NP, and are interested in heart failure
• You assess complex issues in a structured manner, set priorities appropriately to the situation and independently derive suitable measures from them
• You advise employees in a target group-appropriate manner and also act professionally and reliably in medical or organisational matters
• In interprofessional collaboration, you actively contribute your professional perspective and help shape new structures and processes in a solution-oriented manner
