- Consultation of our inpatient patients and their relatives
- Evaluation and implementation of follow-up solutions
- Organization of external care (support services) upon discharge
- Referral to further counseling and contact points
- Clarification of social insurance issues
- Support with adult protection measures
You convince us with this profile:
- Completed training as a social worker FH or further training in case/patient management
- Enjoyment of interdisciplinary collaboration
- Good communication skills
- Knowledge in the field of social insurance
- Solution-oriented personality
- Vacation replacement in the team
- Good PC skills (MS Office)
Preferred working days are Wednesday, Thursday, and Friday.
