Qualified Nursing Specialist with Additional Role as RAI HC Responsible
Your Tasks
- You conduct needs assessments with RAI-HC for clients
- You take on assignments in basic and treatment care for our clients
- You advise and support our clients and their relatives
- You carry out RAI assessments and BMF punctually
- You create care documentation and individual care plans
- Professional support and guidance of the Spitex team
- Planning, coordination and evaluation of care assignments
Your Profile
- Qualified nurse HF or equivalent training
- Experience in outpatient care and good knowledge of RAI HC
- Several years of professional experience in long-term care or Spitex area is an advantage
- You are resilient, flexible and used to working independently
- You are a strong communicator and a responsible personality with team spirit
