Your tasks:
- Carrying out and ensuring professional care and support of our clients in their own homes
- Planning and documenting care processes
- Supporting clients in managing everyday life and promoting their independence
- Close cooperation with general practitioners, therapists, and other persons involved in care
- Advising and supporting relatives
Your profile:
- Completed training as an HF nurse or a comparable qualification
- Experience in outpatient care is an advantage
- High professional and social competence as well as empathy
- Category B driving license
- Enjoy working with people and in an interprofessional team
