Your Tasks
- Analysis and documentation of the home, social and professional situation of the rehabilitand as well as responsible preparation of parts of the final report
- Advising patients on essential social law matters and supporting them with applications
- Support in coping with illness and organising realistic assistance offers
- Cooperation with all employees of the interdisciplinary team
- Independent presentations to patients and rehabilitands
- Group leadership in social skills training
- Participation in the establishment of the MBOR structure in the facility
- Accompanying and supporting our rehabilitands in regaining the ability to act in their own home
- Contact and maintenance of relationships with employers, social welfare offices, care facilities and services, cost bearers and follow-up therapists
Your Profile
- Completed degree in Social Work with state recognition
- You have methodological approaches to activating and accompanying assistance for people with predominantly orthopaedic and psychosomatic illnesses
- Computer skills
- Independent and reliable working style
- Empathy, organisational talent and strong communication skills
- Ability for professional and multiprofessional exchange
- Resilience and ability to work in a team
- Co-responsibility for the continuous improvement of therapy processes and customer satisfaction
- Experience in clinical work, ideally also in rehabilitation, is desirable but not a prerequisite for employment
