The Ostermundigen Medical Centre is part of the Bärn-Netz, an integrated care region in the Greater Bern area. Within Bärn-Netz, general practices, specialists, hospitals, therapeutic services, nursing, and home care services work closely together to better support patients along their health journey. The goal is coordinated, local, and high-quality care, with strong primary care as the central anchor.
Within this network, VIVA is developing an alternative basic insurance model focusing on prevention, health coordination, chronic care management, and personal support. Health coordination assists patients and general practitioners in daily life, provides orientation within the care system, and helps avoid unnecessary duplication, emergency consultations, and hospitalisations.
To strengthen our team, we are looking for a committed APN Nurse Practitioner. You will work closely with doctors, therapists, the nursing team, and an already integrated APN, actively developing the APN role in primary care and for the new basic insurance model VIVA.
Job Description
- Conducting anamneses and clinical examinations as well as holistic assessment of nursing, medical, and social needs
- Clinical reasoning, initiating additional diagnostic investigations, and formulating initial medical and nursing (suspected) diagnoses, if necessary in consultation with the responsible doctor
- Planning and implementing appropriate nursing and medical measures based on the findings
- Medication management including prescription, review, and adjustment within the scope of competencies, as well as educating patients on safe medication use
- Conducting consultations, advice, and training in prevention, health promotion, and self-management
- Supporting patients and relatives with chronic illnesses and promoting adherence and health literacy
- Initial assessment of acute situations, evaluating urgency, and initiating necessary measures or early involvement of medical staff
- Interprofessional collaboration with internal and external specialists and active participation in shaping care processes
- Recognising ethical issues and contributing to joint, patient-centred decisions
- Independent action within the defined scope of tasks and competencies, as well as continuous reflection and development of one’s own role
Qualifications
- Completed Master of Science in Nursing (MScN), ideally with a focus on Nurse Practitioner
- Preferably professional experience in primary care and/or health coordination
- Very good knowledge of medical terminology and confident German language skills, both spoken and written
- Good Microsoft Office skills, especially Word, Excel, and Outlook; experience with hospital or practice information systems is an advantage
- Independent, responsible, and reliable working style with a high quality standard
- Initiative, resilience, reflective ability, and strong organisational skills
- Networked thinking and a solution-oriented working approach
- Enjoyment of interprofessional collaboration as well as confident, empathetic, and service-oriented communication
- French and English language skills are an advantage
