Your tasks
- Outreach care, counselling and psychosocial support for people with mental illnesses
- Promotion of everyday coping, social participation and independent living
- Support with leisure activities as well as referral and accompaniment to further help and support services
- Individual care planning and needs-based care management
- Promotion of social integration and participation in community life
- Professional documentation, reporting and progress documentation
Your profile
- Completed studies in Social Work or Social Pedagogy (Diploma, Bachelor or comparable degree)
- Professional experience in the care and support of people with mental or emotional illnesses desirable
- Independent, structured working style as well as strong team and communication skills
- Confident handling of common IT applications and digital documentation
- Driving licence class B for outreach care and support in the outpatient sector
