Your tasks
- Contact person for clients, relatives and employees on all questions concerning dementia
- Ensuring a high quality of care for our clients at home
- Needs assessment according to interRAI up to the completion of the care process as the case-leading nursing specialist
- Taking on nursing and administrative tasks
- Interdisciplinary collaboration with employees, specialist agencies and doctors
- Project work and taking on additional tasks
Your profile
- Training as an HF nursing specialist
- Ideally with further training in the field of dementia or willingness to further train in this area
- Professional experience in the outpatient sector is an advantage
- Appreciative communication culture and command of the German language, both spoken and written
- interRAI HC course or willingness to complete it
