Your tasks

• Consultation and professional support of caregiving relatives – by phone, digitally and during home visits
• Comprehensive assessment of individual care and support needs
• Development of suitable solutions together with clients and their relatives
• Long-term support and assistance with the aim of promoting independence and quality of life
• Close cooperation and coordination with general practitioners, therapists, insurance companies, social services and other specialist agencies
• Participation in case discussions and internal team meetings
• Careful documentation of care processes, services and observations in the electronic system
• Ensuring correct service recording
• Collaboration in the further development and optimisation of care and support processes
• Participation in internal projects, quality developments and training

Your profile

• Completed training as a Dipl. Nursing Specialist HF/FH or an equivalent recognised qualification
• Experience in outpatient care, nursing consultation or care for relatives is an advantage
• Enjoyment of consultation and personal contact with clients and relatives
• Independent, structured and responsible way of working
• Strong communication skills and empathy
• Confident handling of digital work tools and electronic nursing documentation
• Good German language skills, spoken and written
• Driving licence category B
