About the job

Awareness of the importance of a reliable contact person for children and adolescents with complex chronic illnesses as well as for their families – in everyday life, in crisis situations, at the end of life, and beyond

Recognizing, triaging, and accompanying acute crisis situations (including organizing home visits and other interventions)

Families and treatment teams are networked and advised with the involvement of local resources (pediatricians, children's hospice care, therapeutic services, and institutions); the flow of information throughout the care network is ensured

Planned hospitalizations are prepared and transitions – especially discharges to the home setting – are accompanied

Individual care plans are created and implemented together with those involved

Home visits are conducted and complex decision-making processes are accompanied interprofessionally

Roundtables and other discussions are moderated after appropriate training

Ensuring the rotating 24/7 on-call service within the framework of professionally competent telephone emergency advice in close coordination with the medical background service

From September 1, 2026, or by agreement, we are looking to strengthen the Pediatric Palliative and Advanced Care Team with a Nurse Specialist for Pediatric Palliative and Advanced Care (40-60%).

Your tasks

Your profile

Successful completion of training as a certified nurse HF or FH with several years of experience in the pediatric field
Further training in Pediatric Palliative Care (Level C, CAS, MAS, or equivalent training)
Willingness to support the team's 24/7 on-call service (with medical background)
A cheerful and helpful personality with a strong service orientation and organizational talent is required, as well as the ability to work independently
Mobility (driver's license and own vehicle) is an advantage
