About the job

  • Awareness of the importance of a reliable key person for children and adolescents with complex chronic illnesses as well as for their families – in everyday life, in crisis situations, at the end of life and beyond
  • Recognising, triaging and supporting acute crisis situations (including organising home visits and other interventions)
  • Families and treatment teams are networked and advised with the involvement of local resources (paediatricians, children’s home care, therapeutic services and institutions); the flow of information throughout the care network is ensured
  • Planned hospitalisations are prepared and transitions – especially discharges to the home setting – are supported
  • Individual care plans are created and implemented together with those involved
  • Home visits are carried out and complex decision-making processes are supported interprofessionally
  • Round tables and other discussions are moderated after appropriate induction
  • Ensuring the rotating 24/7 on-call service within the framework of professionally competent telephone emergency advice in close coordination with the medical on-call service

    • From 1 September 2026 or by arrangement, we are looking to strengthen the Paediatric Palliative and Advanced Care Team with a Nurse Specialist for Paediatric Palliative and Advanced Care (40-60%).

    Your tasks

    Your profile

    • Successful completion of training as a qualified nurse HF or FH and several years of experience in the paediatric field
    • Further training in Paediatric Palliative Care (Level C, CAS, MAS or equivalent qualification)
    • Willingness to support the team’s 24/7 on-call service (with medical background)
    • A cheerful and helpful personality with a strong service orientation and organisational talent is required, as well as the ability to work independently
    • Mobility (driving licence and own vehicle) is an advantage
