Dipl. Nursing Specialist HF as Case Coordinator (70-100%)
Your tasks
In this versatile role, you combine professional nursing with coordination and case management:
- You care for and support clients on site as part of nursing assignments.
- You coordinate and organise service delivery from admission to discharge – in close collaboration with clients, relatives, doctors, hospitals and other specialist agencies.
- You take responsibility for the entire nursing process including needs assessment, nursing diagnosis according to NANDA and electronic nursing documentation.
- You delegate tasks appropriately according to the situation and competence and support employees with professional questions.
- You take daily responsibility and contribute to high nursing quality.
- You actively participate in the training of apprentices.
What you bring with you
- Diploma as Nursing Specialist HF or an equivalent qualification
- Experience in needs assessment with InterRAI is an advantage
- Enjoyment of direct nursing as well as coordinative tasks
- High organisational and communication skills
- Independent, responsible and solution-oriented working style
- Openness to change and new developments
- Team spirit, commitment and enjoyment in contact with people
