Life & Living «In der Au» is the guiding principle of VitaFutura AG and includes living, care, catering through the in-house kitchen as well as the public restaurant & bistro LaVita. For the qualified implementation of these services, VitaFutura AG currently employs 140 staff in the areas of nursing, Spitex, gastronomy, technical service and administration. The company operates as a non-profit joint-stock company on behalf of the municipality of Volketswil.

We are looking to complement our Spitex team with a: Dipl. Nurse HF with Case Management - Spitex 60-100% Workload 60% - 100% Your tasks

• Independent and professional nursing and care of clients in their own homes
• Responsibility for care planning as well as assessment and adjustment of care processes and their documentation in the nursing records
• Maintaining nursing documentation and service recording according to applicable guidelines
• Responsibility for case management of assigned clients
• Participation in quality development and continuous improvement of care processes

Your profile

• Completed professional training as a nurse HF/FH or an equivalent foreign diploma recognised by the SRK
• Service- and person-oriented approach as well as a high degree of empathy and professionalism
• Independent, organisational talent, responsible and goal-oriented
