Your tasks 

• Independently manage a varied daily routine in outpatient care
• Ensure high quality of care
• Act as a link between employees, management and other professionals
• Provide professional guidance and daily support to colleagues          
• Interdisciplinary collaboration and coordination with external agencies
• Opportunity to take on case management and case responsibility

Your profile

• Certified nurse HF or FH
• Professional experience in outpatient care
• Motivation to initiate new things and maintain proven practices
• Open, communicative and empathetic manner in dealing with colleagues, clients and relatives
• Driving licence category B or enjoyment of cycling
