RAI Nurse - HC (M/F)
Activity rate between 80%
Permanent contract
Your mission
- Carry out assessments and reassessments of all client situations at their home, using the RAI-HC method (Resident Assessment Instrument - Home Care)
- Evaluate situations and identify needs
- Plan, coordinate and provide care and ensure follow-up
- Advise and inform the cared-for persons and their relatives
- Contribute to maintaining health and preventing illness through an interdisciplinary approach
- Strengthen the autonomy and adaptability of the person
- Develop the quality of care and adapt services according to new knowledge
Your profile and skills
- Bachelor in nursing or equivalent qualification (foreign diploma recognised by the CRS)
- RAI-HC evaluator training is an asset
- Proven experience of 2 years in the medico-social field, in interdisciplinary collaboration and networks
- Sense of responsibility, ability to organise autonomously and set priorities
- Ability to collaborate in networks and multidisciplinary teams
