Qualified Nursing Specialist HF (60% - 100%) - with Development Opportunities

Your tasks:

• Independent and competent execution of outpatient care
• Assuming case management and responsibility for the care process
• Conducting needs assessments with InterRAI HC
• Planning, coordination and evaluation of care
• Consultation and support of clients and their relatives
• Collaboration with relatives, doctors and other professional bodies
• Participation in the further development of care quality
• Opportunity to take on additional specialist or leadership responsibilities

What you bring to the position:

• Diploma as a qualified nursing specialist HF
• Experience in outpatient care is an advantage
• Enjoy working independently and responsibly
• Interest in further developing professional knowledge and quality
• Appreciative and solution-oriented communication
• Strong social skills
• Driving licence category B, as well as own car, scooter or e-bike
