Your Tasks
- Conducting structured patient conversations and professional patient care
- Independent observation and communication of therapy success
- Creation, evaluation and adjustment of care and treatment planning
- Proper documentation in the electronic patient record
- Co-design of the weekly plan and coordination in the multiprofessional team to evaluate the course of treatment
- Promotion of patients' social and emotional skills (m/f/d)
Your Profile
- Completed training as a qualified nurse (m/f/d), curative education nurse (m/f/d) or geriatric nurse (m/f/d)
- Appreciative and patient-oriented basic attitude
- Strong creativity, teamwork skills, flexibility
- High commitment and pronounced sense of responsibility
