Job description
La Plaine Residence is part of the Mandement EMS and houses 92 residents. We support people of AVS age who are losing autonomy and sometimes suffer from psychological or psychiatric disorders. Our philosophy is based on multidisciplinary collaboration.
As such, we are looking for a Case Manager Nurse on a 70% permanent contract.
Mission

You will be responsible for ensuring the coordination, smoothness, and quality of the admission and integration process of residents, from the first contact to the integration assessment, guaranteeing optimal collaboration between the various services (care, hospitality, activities, administration).
Coordination of the admission process
- Receive and analyse admission requests.
- Initiate contact with clients, their key relatives or liaison agents (HUG, CMS, hospital, curators, social workers, etc.) and identify/search for missing documents to ensure the viability of the file.
- Organise and conduct facility visits.
- Organise clinical visits (at the place of living or stay).
- Present and have administrative and financial files validated by general management.
- Present and have medical files validated by the care management.
- Have files validated by the structure’s attending physician.
- Plan, announce and coordinate the admission in collaboration with the structure’s managers (logistics, technical service, activities).
Initiation of the Individualised Support Project (PIA) from integration
- Initiate the PIA of the new resident by integrating their specific needs during their reception in the structure.
- Consider the different aspects of support.
- Prepare the medical file, anticipate medico-technical needs, medical and paramedical follow-up.
- Arrange the room and take preferences into account in coordination with the logistics and technical services.
- Provide information on activities and specific needs of the future resident to the activities service.
Resident pathway follow-up and handover
- Ensure regular follow-up during the first weeks.
- Plan and participate in the integration assessment.
- Plan and ensure the handover to the referents for the first PIA appointments.
- Supervise subsequent PIA appointments (at least one annual meeting).
- Initiate and document the new resident’s disability pension (API) request six months after admission.
Specificities
- You will be required to work across the group’s three facilities.
- You may be asked to take over the PLAISIR evaluator role at the management’s request.
- You may be asked to take a nurse position according to service needs.
Profile sought
Mandatory criteria
- Experience in geriatrics and psychogeriatrics or a similar setting.
- Holder of a Bachelor’s degree in nursing.
- Holder of the PLAISIR evaluator training.
- Excellent mastery of EMS admission processes.
- Excellent level of French, both written and spoken.
- Analytical and writing skills.
- Availability for flexible hours, including weekends and public holidays.

- Knowledge of Sadies DIR software (an asset).
- Empathy, patience and kindness.
- Excellent communication and listening skills.
- Ability to communicate clearly, precisely and appropriately with the interlocutor.
- Adaptability and team spirit while being autonomous and organised.
- Positive attitude and able to demonstrate collaborative leadership.
- Strong sense of rigour and responsibility.
- Capable of managing priorities, taking initiatives and collaborating within a multidisciplinary team.
- Strong interest in elderly people with cognitive disorders.
