About the job
Your Area of Responsibility
- Advising and supporting patients and their relatives in social matters
- Arranging the necessary social services
- Organising the hospital discharge of patients
- Promoting cooperation with other social institutions, regional social services and
nursing or care homes for the elderly or disabled
Your Competences
- Completion of training/studies in nursing at tertiary level or comparable training/studies
- Training in Case Management or willingness to complete it
- Preferably already professional experience in a similar institution
- Balanced personality with high social competence, great willingness to serve, and the ability to remain calm, keep an overview and focus on solutions even in hectic situations
- Independent working style, responsible and high willingness to make decisions
