What you will do with us:
- You take responsibility for the holistic case management of your clients.
- You coordinate and manage the care process with interRAI HC.
- You conduct needs assessments as well as professional and case discussions.
- You document care services and relevant information carefully and digitally.
- You coordinate collaboration with relatives, general practitioners, hospitals and other partners.
- You take on daily responsibility and support a smooth workflow within the team.
What you bring with you:
- You have completed training as a Nurse HF/FH.
- You work independently, responsibly and in an organised manner.
- You possess high professional and social competence.
- You have a category B driving licence.
