For our Memory Care Team at the Albisrieden location, you are a valuable part of our clients’ daily lives. You actively shape their daily experiences and bring warmth, attention, and professionalism to every encounter. Your working hours are from 7:00 am to 4:00 pm.
To strengthen our teams, we are looking for motivated support in the Memory Care Team from 1 November 2026. After six months, a location change may be possible – you will receive more detailed information in a possible initial interview.
Qualified Nurse with Case Management - Albisrieden Location 60-100% (a)
YOUR TASKS IN THE MEMORY CARE AREA
- Case management with a special focus on the individual care of people with dementia, including planning, control, and evaluation of the care process
- Needs assessment and goal-oriented care taking into account memory profiles and daily routines
- Empathetic advice, support, and assistance for relatives in dealing with dementia
- Interdisciplinary teamwork within the Skill & Grade Mix framework, focusing on the special requirements of Memory Care
- Careful documentation of care, behaviour, and cognitive changes
- Introduction and support of new employees in dealing with people with dementia
- Coordination with specialist agencies, referrers, and doctors

YOU BRING WITH YOU
- Completed nursing training at tertiary level
- Good knowledge of the German language, spoken and written, at least level B2
- Driving licence category B or willingness to travel by e-bike
- Friendly and appreciative demeanour as well as physical and mental resilience
- Enjoyment of change and teamwork as well as a high sense of responsibility, independence, and flexibility
