Dipl. Nursing Specialist HF/FH with Case Management 50% - 90%

This is your field of activity

• From the initial visit with needs assessment to the completion of the care process, you competently accompany our customers through their daily lives
• As the person responsible for the day, you support the team with advice and assistance in case of questions
• You demonstrate your advisory competence both in contact with relatives and in collaboration with health professionals and nursing assistants
• With your organisational talent, you act as an interface in interprofessional collaboration
