Role Description
- Conduct the assessment of social situation, follow-up, and social support for patients of our Toises psychotherapy center, taking into account the psychiatric dimension
- Work within a multidisciplinary team consisting of psychiatrists, psychologists, nurses, neuropsychologists whose mission is to ensure integrated psychiatric and psychotherapeutic follow-ups
- Coordinate the social care of the patient in collaboration with the various stakeholders of the center and with the network (CMS, CSR, SCTP, DGEJ, etc.)
- Conduct educational and support interventions in concrete steps (economic, administrative, professional, relational, legal, assistance in finding housing, searching for a place in an institution)
- Assume the role of administrative referent, pending the establishment of an adult protection measure by the Justice of the Peace, and collaborate with the patient to avoid the loss of social gains
Your profile
- Bachelor's degree in Social Work, social service orientation or equivalent title
- Professional experience in psychiatry, an asset
- Competence in assessing the social situation of patients presenting mental disorders
