Your field of activity

• You are responsible for optimal care and advice for our clients with a focus on somatic care
• With assessment and case management, you take responsibility for all steps in the care process of your clients
• You take on coordination tasks with other involved services and actively participate in collaboration with our interprofessional team

• Completed training as a qualified nursing specialist (DNII / HF / FH)
• You are an empathetic, innovative, flexible and cheerful personality with pronounced social, communication and self-competence skills
• You hold a driving licence category B
