Certified Nurse HF 40-100%, with additional function RAI-HC Responsible

Your Tasks

You conduct needs assessments with RAI-HC at the client
You take on assignments in basic and treatment care with our clients
You advise and support our clients and their relatives
You carry out RAI assessments and BMF on time
You create care documentation and individual care planning
Professional support and guidance of the Spitex team
Planning, coordination, and evaluation of care assignments

What You Bring

Certified nursing specialist HF or an equivalent qualification
Experience in outpatient care and good knowledge in the area of RAI HC
Several years of professional experience in long-term care or Spitex area is an advantage
You are resilient, flexible, and used to working independently
You are a communicative and responsible personality with team spirit
