About the job
Spitex Foppa is active in the care and support of the 6 municipalities in the catchment area of Ilanz. The catchment area includes Ilanz/Glion, Lumnezia, Obersaxen Mundaun, Safiental, Vals and Breil/Brigels (Andiast/Waltensburg). We care for and support clients in their familiar environment according to the reference care model.
We are looking for immediately or by arrangement

Your area of responsibility:
- Leading the care and support team
- Promoting employees in providing the service and ensuring the flow of information
- Supporting care management and planning
- Care and support of clients at home
- Conducting needs assessment / ensuring care quality / case management
You bring with you:
- Completed professional training as a certified nursing specialist HF/FH
- Leadership training or willingness to complete it
- High professional and social competence
- Independent, responsible and organized way of working
- Good IT skills in common MS Office applications,
perigon experience is an advantage
