About the job
Company Description
The healthcare sector faces significant challenges. Social, demographic, and health policy changes require well-coordinated services to ensure the quality of medical care is maintained in the future.
The Ärztezentrum Ostermundigen is part of the Bärn-Netz, an integrated care region in the Greater Bern area. Within Bärn-Netz, general practices, specialists, hospitals, therapeutic services, nursing, and home care services work closely together to better support patients along their health journey. The goal is coordinated, local, and high-quality care, with strong primary care as the central anchor.
Within this network, VIVA is developing an alternative basic insurance model focusing on prevention, health coordination, chronic care management, and personal support. Health coordination assists patients and general practitioners in daily life, provides orientation within the care system, and helps avoid unnecessary duplication, emergency consultations, and hospitalisations.
To strengthen our team, we are looking for a committed APN Nurse Practitioner. You will work closely with doctors, therapists, the nursing team, and an already integrated APN, actively developing the APN role in primary care and for the new basic insurance model VIVA.
