To strengthen our team, we are looking for a

Certified Nursing Specialist HF / FH (40-100%)

Your diverse tasks are:

You are active in direct nursing and take on the advice and guidance of clients, relatives and employees in complex care situations
You carry out needs assessments using the InterRAI instrument
(Costs for InterRAI training are covered by RegioSpitex Limmattal)
As case manager, you are responsible for planning and implementing the nursing process for your clients
You play an active role in collaboration with the supervising interdisciplinary network and coordinate the corresponding services for your clients

You bring with you:

You have a qualification as a Certified Nursing Specialist HF / FH and ideally relevant professional experience in this field
You are an open and flexible personality who enjoys engaging with people
You have good communication skills and are confident in using mobile nursing documentation (smartphone)
You think in a networked way, work independently and responsibly
