• Efficient and sustainable discharge planning to increase the satisfaction of patients and relatives as well as to reduce readmissions and rehospitalisations
• Conducting interdisciplinary discussions with patients, relatives and professionals
• Planning and organising transfers to rehabilitation, cures, nursing homes or care in the familiar environment
• Advising inpatient and outpatient patients as well as their relatives on psychosocial, financial and social insurance issues
• General administrative support and coordination for the entire social services team

• Training in social work or further education in case management
• Ideally several years of professional experience in nursing
• Experience in advising people in stressful life situations
• Strong communication and coordination skills
• Knowledge in the field of social insurance
