Social Assistant for the Outpatient Psychiatry Unit of Yverdon
In order to strengthen the psychosocial support of patients followed on an outpatient basis, the North Adult Psychiatry and Psychotherapy Service (SPPAN) is looking for a social assistant, at a rate of 50% to 60%, for the Outpatient Psychiatry Unit (UPA) of Yverdon-les-Bains, within the framework of an open-ended contract.
The starting date is scheduled for April 1, 2026, or to be agreed upon.
Context
The Lausanne University Hospital (CHUV) is one of the five Swiss university hospitals. Thanks to its collaboration with the Faculty of Biology and Medicine of the University of Lausanne and EPFL, it plays a leading role in medical care, research, and education.
The North Adult Psychiatry and Psychotherapy Service (SPPAN) provides care for adults aged 18 to 65 residing in the northern Vaud region. Its approach is based on close collaboration with the care network to offer comprehensive, coordinated, and individualized care.
The North Vaud Psychiatric Center (CPNVD), affiliated with CHUV, brings together the hospital and outpatient psychiatry units of Yverdon-les-Bains, Orbe, and Payerne.
Integrated at the heart of the care system, the social service supports patients in developing individualized care plans, in connection with relatives and social, institutional, and community partners.
Mission
- You conduct psychosocial assessments and ensure follow-up of patients presenting social issues
- You identify and analyze, with the patient and/or their entourage, difficulties in daily life
- You seek, in collaboration with the medical-nursing team and the patient, solutions adapted to the situation
- You carry out accompaniment procedures in the community and with network partners (institutional visits, home visits, CSR, justice of the peace, etc.)
- You inform, guide, and support the patient and their relatives in administrative, professional, legal, and relational procedures
- You participate in the development of housing projects, in partnership with the multidisciplinary team
- You investigate and evaluate the role and skills of actors in the patients’ primary and secondary social networks
- You collaborate closely with the multidisciplinary team, SPPAN partners, and the Vaud medico-social network.
Profile
- You hold a Bachelor of Arts HES-SO in social work or an equivalent qualification, with SEFRI recognition mandatory for foreign diplomas
- You have experience working with people presenting mental disorders
- You have good knowledge of social insurance
- You know the Vaud socio-health network, medico-social institutions, administrative and legal structures, as well as housing arrangements
- You demonstrate autonomy, initiative, and creativity in finding solutions
- You possess good analytical and synthesis skills
- You know how to practice active listening and communicate effectively
- You appreciate teamwork and show openness and flexibility
- You hold a driving license and have a vehicle
Depending on your profile, the salary class will be between level 8 and level 9.
