Your tasks
- Organisation of discharge management
- Clarification of financial and structural problems of patients and organisation of appropriate assistance
- Committed and self-responsible organisation of the daily work routine
- Representation of the clinic internally and externally as well as maintenance and expansion of existing cooperations and networks
- Participation in multiprofessional team meetings and close cooperation with other specialist departments
- Participation in working groups, e.g. the state working group of social services of psychiatric clinics in S-H
Your profile
- A completed degree in social work or social pedagogy (Bachelor, Diploma or Master)
- Openness, respect and interest in dealing with people with mental illnesses
- Willingness to build networks through cooperation with, for example, outpatient assistance, inpatient integration facilities
- You enjoy interdisciplinary teamwork
