Your Tasks
- You are responsible for the comprehensive and individual management of the patient process from admission to discharge through professional support of the patients. Patient-related problems as well as economic aspects are taken into account. The patient coach is the central contact person for all interfaces and coordinates them
- You advise patients, relatives, and the interprofessional team on specific topics and problems
- With the referrers (family doctor, home care, nursing home) in the emergency department, you clarify under which conditions the patient can be transferred back or directly moved to a follow-up solution without inpatient admission
- Consultations and clarifications in psychosocial, legal, financial, and economic situations related to the hospital stay are part of your tasks
- You organize the desired/required follow-up solution, clarify the financing situation, and/or obtain the necessary cost approvals
- In addition, you participate in defining common interdisciplinary care goals and implementing appropriate measures and resources
Your Profile
- You have a completed degree in Social Work
- Ideally, you bring experience in clinical social work and/or knowledge in the medical or nursing field
- You are willing to expand your skills in one of our private hospitals and deepen your expertise further.
- You recognize difficult situations in time and can therefore prioritize correctly.
- You enjoy contact with people and treat them with respect, appreciation, and good manners.
