Certified Nursing Specialist HF 60-100% (optional: with additional function RAI-NH Responsible)

Your Tasks

You conduct needs assessments with RAI-NH at the client (optional)
You take on assignments in basic and treatment care with our clients
You advise and support our clients and their relatives
You carry out RAI assessments and BMF on time
You create care documentation and individual care planning
Professional support and guidance of the home care team
Planning, coordination, and evaluation of care assignments

What You Bring

Certified nursing specialist HF or an equivalent qualification
Experience in outpatient care and good knowledge in the area of RAI NH
Several years of professional experience in long-term care or home care is an advantage
You are motivated to optimize existing processes and build new structures
You are resilient, flexible, and used to working independently
You are a communicative and responsible personality with team spirit
