About the job
For our team with a focus on consultative, geriatric and neuropsychiatry (KAN), we are looking for you immediately or by arrangement as
Tasks
- You advise patients and their relatives regarding suitable living arrangements and daily structure as well as social insurance law, financial and adult protection law issues. In doing so, you are in close contact with the involved professionals
- You promptly record the needs of newly admitted patients and their relatives and jointly seek suitable and sustainable follow-up solutions for discharge from the clinic. You orient yourself to legal and economic framework conditions and shape the available scope of action in the interest of the patients
- You contribute your expertise to the interprofessional treatment team, namely from medical, psychological and nursing professionals, and actively support the achievement of treatment goals
- You participate in reports and case discussions
Profile
- Bachelor of Science / Arts from a university of applied sciences for social work
- Professional experience in working with older people or a comparable environment
- High presentation skills, service orientation, initiative, flexibility, independence and resilience
- Interested and empathetic personality with joy and motivation for networked work
- Your strengths include a positive view of humanity, social skills, solution orientation as well as a healthy dose of humor and pragmatism
- You are a team player with a high sense of responsibility and quality awareness and are used to setting priorities and working independently
- You place dignity, self-determination and social participation of older people at the center of your work and can recognize and professionally deal with the associated opportunities and limits
