Certified Nurse with HF with RAI Responsibility 50-100% (half working days also possible)

Your Tasks

You will take on assignments in basic and treatment care with our clients
You advise and support our clients and their relatives
You conduct needs assessments with RAI-HC at the client’s location
Home care case management and active participation in expanding our internal home care services
You create care documentation and individual care planning

What You Bring

Training as a certified nurse
Several years of professional experience in long-term care or home care is an advantage
You are resilient, flexible, and used to working independently
You have completed the RAI Supervisor or RAI Expert training
