Your tasks
- Social counseling of patients and relatives in the areas of work/daily structure, housing, finances, law/social insurance, and social environment
- Collection of social history and diagnosis as well as implementation of planned intervention steps in close coordination with the interprofessional treatment plan
- Support of rehabilitation and reintegration of patients into social systems as well as networking with relevant external agencies
- Dealing with the connections between social problems and mental illnesses as well as with social work interventions that contribute to health stabilization and recovery
- Conducting conversations and record keeping, correspondence, and documentation
