Job Description

• You work directly in nursing and take on the advice and guidance of clients, relatives and employees in complex care situations

• You carry out needs assessments using the InterRAI instrument

• As case manager, you are responsible for planning and implementing the care process for your clients

• You play an active role in collaboration with the supervising interdisciplinary network and coordinate the corresponding services for your clients

Requirements Profile

• You have a qualification as a Dipl. Nursing Specialist HF / FH and ideally corresponding professional experience in this field

• You are an open-minded and flexible personality who enjoys engaging with people

• You bring good communication skills as well as confident handling of mobile care documentation (smartphone)

• You think in a networked way, work independently and responsibly
