Dipl. Nurse HF/FH with Case Management (80-100%)
Can you imagine working in a small team without a direct supervisor? We have reorganised into semi-autonomous teams, meaning we are guided by the famous Buurtzorg model, but have adapted it to our needs. In an interdisciplinary team, with case managers, nursing assistants and domestic and care staff at your side, it is intended to be a new way of working that is contemporary and allows necessary flexibility, but also structure and continuity.
You have the following qualifications:
- completed training as a dipl. nurse HF/FH
- ideally Spitex experience
- strong professional and social skills
- independent and responsible way of working
- valid driving licence category B
Your tasks: As a case-managing employee, you control the care process from the initial visit with needs assessment according to interRAI to completion. You are the interface for interprofessional collaboration with all involved parties, advise relatives and professionally support healthcare professionals as well as the care team. You provide holistic and resource-oriented care to our clients.
