This is your task
In the role of IHP Coordinator, you are responsible for the process from registration, through needs assessment, to receiving the IHP decision. In addition, you help to develop and establish the new structures for implementing and documenting the commissioned services.
Together with the IHP specialists, you support and accompany the residents throughout the entire IHP needs assessment process and regularly check the currency of the individual needs. In doing so, you maintain close contact with internal and external specialists. You also share your knowledge and experience within the interdisciplinary team.This is what you bring
You have a completed degree in social work (or a comparable qualification) and bring professional experience in supporting people with disabilities and ideally in the IHP process. Networked thinking, strong communication skills and organisational talent complete your profile.
With your open, straightforward and empathetic manner, you easily engage with the residents and gain their trust. Your high professional and social competence and your sense of responsibility distinguish you and are a great asset in cross-departmental collaboration.
