Qualified Nurse with Case Management 50-100%
Your Tasks
- You will carry out assignments in basic and treatment care with our clients
- You advise and support our clients and their relatives
- You conduct needs assessments with RAI-HC at the client’s home
- You create care documentation and individual care plans
- You instruct, advise and support other staff members
- You take on holiday cover for the nursing service management
Your Profile
- Training as a qualified nurse
- Several years of professional experience in long-term care or home care is an advantage
- You are resilient, flexible and used to working independently
