Your Tasks:

• Assessment and consultation of new clients and you professionally accompany them from the start.
• You regularly review the care situation and individually adjust it within the framework of reassessments.
• You actively promote the health, independence, and resources of the clients.
• You work in a structured manner with the software and ensure comprehensive documentation.
• You coordinate collaboration with general practitioners, insurance companies, and other partners.
• You provide advice to relatives and support them in everyday care.

Requirements:

• Completed training as a Dipl. Registered Nurse HF/FH or equivalent
• Recognition (SRK).
• 2 years of professional experience advantageous, ideally in home care or long-term care.
• Independent, responsible, and structured working style.
• High social competence as well as strong communication skills.
• Good German language skills, spoken and written.
• Driving licence category B required.
