About the job
Your Tasks
- Advising and supporting patients in complex social issues as well as taking over case management
- Organising individual and needs-based aftercare and discharge solutions, as well as arranging material assistance and triage to appropriate specialist services if required
- Interdisciplinary collaboration with doctors, nursing staff and other professional groups to ensure coordinated patient care
- Practical training of social work students (BSc) or willingness to complete the corresponding further training
Your Profile
- Completed degree in Social Work (BSc FH)
- Ideally additional training in healthcare (e.g. FaGe, nursing HF or FH)
- Training as a practical trainer or willingness to complete this
- High social and advisory competence as well as enjoyment of interdisciplinary collaboration
- Independent, structured and solution-oriented working style
- Interest in working in the dynamic environment of an acute hospital
