For our specialised professional team, we are looking for a motivated
Nurse HF/FH specialising in Dementia (40 - 80%)
Your tasks:
- You are responsible for the qualified and resource-oriented care and support of our clients as well as their relatives.
- You accompany the entire care process and conduct needs assessments as well as roundtable discussions.
- You coordinate collaboration with other professionals and ensure that relatives and external services are optimally involved.
What you bring with you:
- Completed training as a Nurse HF/FH, ideally with further education in Dementia and/or Palliative Care or willingness to undertake this
- High level of professional and social competence as well as an independent and responsible way of working
- Flexibility and empathy
- Driving licence category B
