- Conduct assessments using interRAI-CMH and manage cases.
- Plan and execute holistic nursing processes independently.
Skills
- Diploma in nursing with psychiatric focus or relevant experience.
- Self-reliant, responsible, and team-oriented work style.
- Reliability and flexibility, driving license preferred.

About the job

Your area of responsibility:
- Psychiatric and psychosocial care and support of chronically and mentally ill people at home
- You conduct needs assessments with interRAI-CMH and take over case management
- Independent implementation and planning of the holistic care process
- You work independently in client deployment but are networked with referrers, psychiatrists / psychologists, and general practitioners.
What you bring with you:
- Diploma in Nursing HF / FH with a focus on psychiatry or professional experience in inpatient psychiatry
- Independent, responsible, and team-oriented working style
- Reliability and flexibility
- Driver's license category B, own car is an advantage
