Tasks

• Outreach acute psychiatric care of patients
• Outreach and integrative case management of mentally ill patients before, during or after an inpatient stay including case documentation as well as writing reports for the insurance
• Work with reference persons including nursing diagnostics
• Interprofessional collaboration within the entire treatment team
• Active participation in the design and implementation of the Hometreatment project as well as in the further development of the service

Profile

• Completed HF or FH degree
• Professional experience in adult acute psychiatry, experience in outreach care is desirable
• Willingness to work in a team, in outreach care and the associated travel activity as well as flexible shift planning (shift work)
• Responsible and proactive working style
• Good German and IT skills
• For foreign diplomas, SRK recognition is required
• Driving licence category B
