At ZURZACH Care, we offer rehabilitation in all its facets at a high medical level. To improve our admission process, we are looking for reinforcement in our rehabilitation coordination team.

Your tasks

• Care assessment: You conduct care assessments before the admission of patients and determine the individual care needs for all locations
• Coordination: You maintain regular communication with the nursing staff to optimally assess care capacities and efficiently plan admissions
• Quality assurance: You conduct telephone patient surveys after discharge to collect feedback on the quality of care and gain valuable insights for further development
• Process development: You actively participate in building the new department of rehabilitation coordinators and continuously develop the process of central care assessment

Your profile

• Education: Completed professional training as a Dipl. Nurse FH/HF or equivalent training with SRK recognition or positive SRK PreCheck
• Experience: At least two years of professional experience in a clinic (rehabilitation or acute hospital preferred) as well as experience in assessing complex care situations and with medical technology
• Skills: Very good German language skills in spoken and written form, strong communication skills in dealing with internal and external partners, as well as enjoyment of administrative tasks and working on the computer
