For our specialised palliative and dementia specialist team, we are looking for a motivated
Nurse HF/FH specialising in Dementia (40 - 80%)
Your tasks:
- Responsibility for care quality: You are responsible for the qualified and resource-oriented care and advice of our clients and their relatives. Your goal is to ensure holistic and empathetic care and support.
- Leadership and management of the care process: You accompany the entire care process, conduct needs assessments and roundtable discussions, and ensure that the quality of care remains consistently high.
- Interdisciplinary collaboration: You coordinate close cooperation with other professionals and ensure that relatives as well as external services are optimally involved.
- Shaping care processes: Your experience and ideas are sought to further develop care processes and continuously improve our care.
What you bring:
- Completed training as a Nurse HF/FH, ideally with further training in Dementia and/or Palliative Care or willingness to further train in these areas
- High level of professional and social competence as well as an independent and responsible way of working
- Flexibility and empathy: You handle physical and psychological stress with confidence.
- Driving licence category B available
