Nurse / Care Specialist Needs Assessment
Your tasks:
- Needs assessment: Conducting needs assessments at clients’ homes to determine care requirements and necessary services.
- InterRAI HC assessments: Applying the InterRAI HC system to carry out standardised needs assessments to ensure precise care and support planning.
- Consultation: Advising clients and their relatives regarding care and support services.
- Creation of care plans: Developing individual care and support plans in collaboration with nursing professionals and the interdisciplinary team.
- Coordination: Close cooperation with nurses, doctors, Spitex staff and other specialist agencies to ensure seamless care provision.
Documentation:
- Good knowledge in creating care plans and needs assessments.
- Strong communication skills and empathy in dealing with clients and their relatives.
- Independent, structured and responsible working style.
- Careful and timely documentation of needs assessments and care planning in the system, especially the InterRAI HC data.
- Quality control: Contributing to ensuring high quality standards in care and implementing further developments in the area of needs assessment.
- Close cooperation with the deputy nursing and care / nursing expert
Your profile:
- Qualified nurse HF or FH (or a comparable qualification in healthcare). RAI expert, InterRAI HC
- Experience in Spitex: Extensive experience in the care and support of clients in the home environment, especially in Spitex.
- Knowledge: Experience in applying the InterRAI HC system for needs assessment and care planning or willingness to familiarise yourself with this system.
- Interest and ability to conduct needs assessments independently and solution-oriented.
- Very good communication skills and empathy in dealing with clients and their relatives.
- Independent, responsible working style as well as high flexibility and resilience.
- Very good IT skills and experience with documentation systems. MS Office, care documentation systems, Perigon experience an advantage
