- Diploma in nursing HF/FH with psychiatric focus or relevant experience.
- Self-driven, responsible, and team-oriented approach.
- Reliability and flexibility, valid driver's license preferred.

Start date by arrangement
Your area of responsibility:
- Psychiatric and psychosocial care and support of chronically and mentally ill people at home
- You conduct needs assessments with interRAI-CMH and take on case management
- Independent implementation and planning of the holistic care process
- You work independently in client assignments but are connected with referrers, psychiatrists/psychologists and general practitioners.
What you bring:
- Diploma in Nursing HF / FH with a focus on Psychiatry or professional experience in inpatient psychiatric care
- Independent, responsible and team-oriented working style
- Reliability and flexibility
- Driving licence category B, own car an advantage
